We service individuals of all ages. The process begins with an initial assessment by one of our professional Case Managers. The Case Manager works with the client and with the client's team of professionals in order to coordinate, advocate, and implement services to maximize client's quality of life. By working together with the client's team of professionals, we strive to achieve and implement the best available solution.
Some of our services include but are not limited to the following:
- Initial assessment of client needs
- Creating Plan of Care
- Home and facility visits and wellness checks
- Arrange and accompany clients to medical appointments
- Assist in hiring and monitoring hired caregivers
- Help coordinate for home modifications (ramps, lifts, elevators, etc.)
- Help select and coordinate purchase of equipment (wheelchairs, beds, handicap vans, etc.)
- Work with school districts for special education needs and attend IEP meetings
- Report to probate court and family members
CASE STUDIES
Case Study #1:
This 62 year old old female was referred for Case Management Services after neighbors found her intoxicated, confused, and combative in her front lawn. She was admitted to a local hospital and ultimately diagnosed with bipolar disorder, depression and alcoholism. History determined that she was alienated from all of her children except a son who lived out of state. No family or neighbors had been in her home for years. Social Services and public health determined the home uninhabitable and the home was condemned.
Over the following 10 years, RNR Case Managers assisted her in maintaining her recovery from alcoholism and assisted her in a continual upgrade of "least restrictive" care facilities until she was able to live in the community.
She found friendship and support with a carefully screened companion hired and trained by her Case Manager. Medication management, physician appointments, cooking, cleaning and shopping were managed and supervised by RNR Case Managers. Free of substance abuse and properly treated for her psychiatric conditions, she was able to communicate with friends and family.
After almost a decade, she chose to relocate out of state so that she could live out her remaining years near her son. Her RNR Case Manager accompanied her and remained involved until appropriate support and care was in place.

Her care included seizure management, gastro intestinal feedings, tracheostomy suctioning, and constant one to one skilled care. Her mother was a single parent with three older children. Case Management became involved when at age 5 her trust officer referred her to us for assistance.
She received a medical malpractice award with funds intended to meet her medical and rehabilitation needs. The RNR Case Manager, after assessing the family dynamics, deemed the mother to be loving and devoted but, exhausted in her efforts to care for her. Nurses were hired to provide direct care. Case Manager recommended and helped her acquire a custom wheelchair, a custom bath seat, a therapy table, positioning equipment and a wheelchair accessible van. The Case Manager arranged for her to have therapies including an assessment for an alternative communication device. With the Case Manager's involvement and assistance she attends school where she receives special education services, including her favorite class, music. She likes to dance in her wheelchair and she is quick to press the button on her electronic touch talker to communicate with others.

RNR Case Managers helped locate a single level townhome and assisted in hiring contractors to create an accessible bathroom, a ramped and enclosed sun room, and a therapy room. Live-in caregivers were hired and trained. Though Monica's goal of returning to her home was met, Case Managers knew that Monica needed a purpose for her days and an opportunity to express her once outgoing personality. A new world was introduced to her in the form of eye-gaze technology.
